PLEASE tell me you didn't already vote before reading this.
So yeah, I feel like I don't really have to explain anything. Here's what each topic includes:
Heroes: You think of your own hero (no using heroes that have already been thought of), his/her exact stats for each level, and his/her skills.
Levels: This one will probably be the most difficult. You must design your own level. You have to make a crude drawing of how the level would like (tower positions, pathways, the shield thingy), you have to explain exactly what and how many enemies (think of new enemies for an A) are in a wave (12 waves btw), and any additions.
Level Specific Additions (positive): These are helpful extras that you see in levels. Stuff like Sasquatch, Sunray, and the Imperial Guards. You must explain in high detail about every aspect. Can you upgrade it? Can you buy more units? Do you have all units you can possibly have when you start?
Level Specific Additions (negative): These are bad bad things that may or may not make you want to scream and ragequit. Repeatedly. Things like the Graveyard and the Green Lake from Rotten Forest. Some examples would be fog, terrain that is dangerous for barracks, or an area with a powerful wind that renders arrows useless. (this category I'm unsure of, I would really like to hear some opinions on whether or not it is a good idea to do this ^^)
