I like the concept a lot. I've been harboring the idea of a hero whose abilities all change their playstyle momentarily and need to be carefully chosen, but figured it wouldn't work well in the game. Here, I see how you go for area damage, durability, increased tower power, or single-target damage output. I have no idea how to make sure she goes into the right mode or how the game would know what I'm planning on doing with the hero, so I think this was the most vital part of the hero that was left out, or at least under-explained.
The powers themselves are nifty, being really cool and having clear purposes. However, some are clearly better than others. Dealing 60 area damage around her is almost as good against one enemy taking 100 damage per attack, so with just 2 enemies it automatically becomes better. Similarly, boosting towers near her allows them to deal massive amounts of damage to the frontal enemies, which would very much outclass the single damage of her ranged ability.
If there was only one thing I could say that you've amazed me with, it would be the hero spell. Way to improve Heavenly Chaos.
I also sense more BFME2 stuff, specifically from the Edain Mod with Galadriel.
The first form seems much better than the 2nd option, in my opinion, and I think they should both be equal, with the third option being the strongest one of all, even though it isn't. Just add some balancing tweaks to help form her playstyle and figure out how to manage those different forms, and she'll be your magnum opus.
